Some people consider Dissection a milestone, some don't, as simple as that, plus I never said milestone, I said the big names jump to mind, IMO Dissection is a big name.

I consider old Mayhem to be dead also. I like the new one, but it isn't the same band that released DMDS, Deathcrush and Live in Leipzig. It has nothing to do with the second/thrid wave of BM (maybe except bandmembers).
So.....? Call them a new band or the same band, it's the same for me, I don't give a fuck about (new or old) Mayhem.

Once again it depends on how you do it, I didn't know there was a standard for this, and the one I see used the most is the one I use:
1st: Venom, Bathory, Celtic Frost bla bla bla
2nd: early/mid 90's
3rd: Nowadays BM.
